Ive heard the following from several nurse friends
Do you have any chronic illnesses?
"Nope"
Are there any medications you take regularly?
"Yeah I take some painkillers for my arthritis."

Like if you have diarrhea as a symptom it's an illness, but if you have reduced mobility it's more like an "injury" or a "condition."
